If you live within our practice area and would like to register at Oakfields Health Centre please ask for a registration form and information pack at Reception.

You will be asked to fill in a form and a simple health questionnaire so that the doctors and nurses have some information about you immediately.

You will be offered an appointment with the nurse or our health care assistant for a new patient “health check” shortly after registering with the practice.

Health checks provide an assessment of your height, weight, blood pressure and discussion/advice about lifestyle such as diet, smoking ,alcohol consumption and levels of physical activity.

If you are on repeat prescriptions for ongoing medicines you will be asked to make an appointment with the doctor. We suggest that you try to obtain a month’s supply of your medicines form your previous practice to give us time to process your new repeat prescriptions.
